Uniqlo Canada - Return Form

If you no longer have your Return Form, please contact our customer service team with your order number and our team will request a new form from our warehouse.
If you have already shipped your return without the return form inside the package, please contact the customer service team with your return tracking number, your order number and which items were included in the returned package.
Order confirmation emails and order history details from your account CANNOT be used to process returns/refunds.
Please note that requesting a replacement return form may take some time during high volume and/or peak seasons. We will do our best to provide the replacement the return form as soon as possible; customers can except to hear back between 1 - 3 business days.
Failure to include a return form will result in in the warehouse not being able to process the return.
